Job Description:
A company is looking for an Analyst, Case Management
Specialist.
Key Responsibilities
Interact with low stratification members via phone, coordinating
care and updating assessments and care plans
Create care plans based on assessments and claims data for members
to follow
Participate in Interdisciplinary Team meetings to discuss members'
healthcare needs and barriers to care
Required Qualifications
3+ years of experience in a health-related field
2+ years of experience with Microsoft Office applications (Word,
Excel, Outlook)
Bachelor's Degree or equivalent experience required
Familiarity with community resources and services
Strong organizational skills and ability to work independently
