Job Description:
A company is looking for a Care Manager (RN) to support DSNP and
MAPD members in California.
Key Responsibilities
Evaluate member needs and develop personalized care plans to
improve healthcare outcomes
Coordinate care between members, families, and healthcare providers
to ensure access to appropriate services
Monitor member progress and update care plans as necessary to meet
individual goals
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree
in Nursing
2 - 4 years of related experience in case management
Active RN license in California or Compact State Licensure
Bilingual in English and Spanish
Ability to work PST hours
