Job Description:
A company is looking for a Hospice Coordinator to join their
team, offering remote or on-site work with flexible hours.
Key Responsibilities
Explains hospice philosophy, levels of care, goals, and services to
patients, families, and staff
Maintains relationships with hospice agency staff to facilitate
communication and transitions of care
Collects outcome and referral data, reporting to leaders on a
regular basis
Required Qualifications
Education: ADN, BSN, or MSW; RN with an Associate's Degree must
complete BSN within 5 years
Experience: Three years in public health/community health,
medical/surgical, palliative, or hospice nursing/SW
Certification: Current RN or SW license in VA or eligible to
practice in Virginia; BLS certification required
Remote work eligibility for candidates residing in specific
states
