Job Description Job Description Full-Time Community Liaison/ Marketer Priority Hospice is a hospice provider located in Covina, CA. We provide hospice services to the eligible patients in the San Gabriel Valley and surrounding areas. Our main objective is to provide quality and safe care to our patient's and their families during life's toughest moments. Our ideal candidate for the Community Liaison position will be knowledgeable in Medicare/Medi-Cal and the Hospice benefit. Previous HOSPICE MARKETING IS A MUST. POSITION SUMMARY The Community Liaison, in conjunction with the other members of the Admission Team, is responsible for activities and actions related to promoting the services and expertise of hospice to the medical community and to the patients and family members. REPORTS TO: Director of Operations SUPERVISES : None QUALIFICATIONS: Education: Background in Healthcare, Marketing or public relations required. Credential: Certified, Licensed or Registered as appropriate. Experience: Two years of Marketing and Healthcare . Core Competencies: Knowledge of hospice requirements including Medicare, Medi-Cal, and private insurance guidelines. Good communication skills. Ability to respond to the needs of physicians, discharge planners, patients and family members. Ability to coordinate services with referral sources, hospice staff and patients and families. Good Marketing Skills. Ability to maintain relationships with referral source. Requirements: Background in Healthcare, Marketing or Public Relations Minimum 2 years of HOSPICE EXPERIENCE Marketing Knowledge of Medicare, Medi-Cal and private insurance guidelines a plus Good communication skills Valid California Driver's License Current auto insurance FUNCTIONS & RESPONSIBILITIES: Responds to inquiries and requests Hospice information in a timely manner. Follows up on referral for evaluation and/or admission to hospice program. Verifies and completes admission and financial information on the referral intake form. Explains the hospice program to patients, family members and other referral sources in an accurate, complete and thorough manner. Provides accurate and timely documentation of all marketing activities to the Director of Operations as requested. Identifies, develops and maintains referral sources. Participates in the creation and implementation of a strategic marketing plan. Organizes and provides education to the community, referral sources and facilities. Demonstrates ability to manage several responsibilities simultaneously and meets productivity standards of the organization. Assignments are completed meeting quality standards such as accuracy, neatness and thoroughness. Demonstrates dependability by adhering to time frames, punctuality, and attendance policies. Takes initiative by seeking greater responsibility and following through appropriately. Demonstrates ability to identify existing problems and communicates with their supervisor. Adapts to changing situations well and handles difficult situations in a tactful manner. Develops knowledge and skills applicable to their role and responsibilities on an on-going basis. Complies with hospice policies and procedures, laws and regulations, standards of practice, and established work rules. Adheres to the highest standard of professional and personal conduct in the performance of his/her duties.
